I've made this based on a reddit thread and will be referencing this fork back on the thread to contribute to it.

There is a "Chat doorbell" created by LX that will be installed directly to their StreamElements account if using this link:

Hilarious. The code and functionality is almost identical to what I made. So the question is.... why can't I actually find it on StreamElement's site when searching if it's already hosted there? The only existence seems to be an unsearchable link and discord. This was a day of wasted effort covering already tread ground. I'm keeping my submission anyway.

Oh, and they forgot to clear their DATA tab in that widget before making it.

Yeah, many of those custom widgets aren't searchable, which is unfortunate. Also, with the deprecation of custom widgets in favour of Elements, I don't think they'll make any progress.
I hope they approve your pull request, but since the repo is abandoned, I wouldn't get my hopes up.

Something broke on the StreamElements end. I'm not getting audio from my own widget anymore. My best guess is that something broke with being able to play the audio from the third party site, since the other widget mentioned still works and that's the only big difference between them. I've fused two scripts together and switched to the audio upload method.

Mishmash of cleaner code from other widget and the functionality of previous version, but fixing the recent breakage of audio by switching to file upload rather than remote URL.
